This happened to me 4 times this morning. Cleared DNS cache, restarted browser, then computer. Kinda sucks I paid for this and I keep getting booted from my modules. I've lost 3 because of "failed attempts".
Sounds to me like it could be an issue with your internet connection. Does your connection have a habit of dying for a second or two? Like if you browse websites, do you often get "page could not be loaded" but refreshing instantly fixes it? I don't think it is possible to reconnect to the server if you lose the connection even for a second. I'm not saying it couldn't be a server issue but if it happens a lot, it might be your connection. Then again I wouldn't be surprised if it was the server because I too had some major lag yesterday, which is something I never get. Also there was of course this new patch that could have messed things up.
One of your images is a broken link. As for the issue, while it seems to happen occasionally, we're not seeing a rampant, constant problem across the board. I'll still push this up the chain and see if there's anything that's happening specifically on our end.
