Queuing for League and Ranked at the same time sometimes gets into a game for both - I started a League game, and some of the graphical elements didn't load, so I restarted the client. When I logged back in, it asked me if I wanted to rejoin my ongoing game... and then put me in an entirely new Ranked PvP game. Apparently, my opponent said that the clock didn't start until I rejoined, but I can't verify that. I lost the League game by default, and lost the Ranked game too, but that wasn't because of the bug
This pops up from time to time, so I think some (stronger) fail safe systems should be added.
