This occurred in Passageways to Death. One of the spiders drew Vampiric Form, the form card attached, and the game froze on Cardotron's turn. I was unable to exit by resigning or adding a co-op partner (I was playing solo). Rebooting the game cleared the freeze, but I wasn't given the option of returning to the battle.
