It happens if it's in your hand at the start of the round. Only way to be rid of it: discard it. This is normally not a problem, though if you're emptying the rest of your hand... well you'll take 1 damage next round. Note that this happens before the draw step.
It's not marked trait as it isn't one. It's a black (drawback) card that has the Keep keyword. It will play at the start of each turn you hold it until you discard it yourself. Traits leave your hand and attach to your character for a period of turns.
Keep means it comes back to your hand after you play it. There isn't anything on the card that says it will play itself. It's a totally cool mechanic; I'm just saying it is confusion level ten.
