Powerful Hack alone wasn't big enough to kill you, and Lifesaving Block didn't see the bonus from Unholy Wellspring. Checking old threads, I see that it should have triggered. Are you certain you were facing the attacker before the attack?
I think you may have it, I don't think I did anything after running away I wasn't thinking about facing as the maniac was chasing me down with his hatchet.
